APPLICATION AND ADMISSION PROCEDURES
Miami Springs Adventist School welcomes applications from young people of all cultural and religious backgrounds who are willing to conduct themselves in harmony with the ideals and standards expressed in our Handbook. Membership in the Seventh-day Adventist Church is not required, but it is understood that everyone who is accepted pledges to observe the regulations of the school.
- Complete the Application Process
- Download and fill out the Student Application for Enrollment/Student Information Verification form.
- Gather two Letters of Recommendation.
- Complete and submit the Dismissal Pick-Up Form.
- Submit Required Documents
- Dress Code Acknowledgement Form
- Universal Field Trip Form
- Computer Loan Agreement
- Video/Photo Release Form
- Transcript Request Form
- Provide a Current Photograph of your child.
- Submit the Current Grade Report and Cumulative Grade Report.
- Obtain Records from Previous Schools.
- Submit any Special/Psychological Education Testing Report (if applicable).
- Provide a copy of your child’s Birth Certificate and Social Security Card.
- Ensure Health Requirements are Met
- Submit the HRS 680 Immunization Form (updated from the physician's office).
- Submit the DH 3040 Physical Examination Form (yellow form from the physician's office).
- Complete the Personal Health History.
Immunizations Required:
- Kindergarten:
- DTP (Diphtheria, Tetanus, Pertussis)
- Polio
- MMR (Measles, Mumps, Rubella)
- Varicella (or confirmation that student has had a history of Chicken Pox)
- Hepatitis B (proof of receiving the first shot and appointments for the next two shots)
- Seventh Grade:
- MMR (Measles, Mumps, Rubella)
- Tetanus Booster
- Varicella (or confirmation that student has had a history of Chicken Pox)
- Hepatitis B (series of 3 shots)
