How to Apply in 3 Simple Steps
Applying to MSAS is straightforward. Here is what your family can expect from start to finish.
Download and Complete the Application Packet
Download the student enrollment packet and complete all required forms. Our office is happy to answer questions while you fill it out.
Submit Records and Required Forms
Submit your child’s academic records, recommendation letters, identification documents, and required school forms before enrollment is reviewed.
Complete Health and Admissions Requirements
Submit current health and immunization records. New students entering grades 2–8 also complete a placement assessment and a brief interview.

Health and Immunization Requirements
Florida school health requirements must be submitted for your child before enrollment is complete.
Required Health Forms
- HRS 680 Immunization Form (updated from the physician’s office)
- DH 3040 Physical Examination Form (yellow form from the physician’s office)
- Personal Health History
Kindergarten Immunizations
- DTP (Diphtheria, Tetanus, Pertussis)
- Polio
- MMR (Measles, Mumps, Rubella)
- Varicella, or confirmation of Chicken Pox history
- Hepatitis B (proof of first dose and appointments for the next two)
Seventh Grade Immunizations
- MMR (Measles, Mumps, Rubella)
- Tetanus Booster
- Varicella, or confirmation of Chicken Pox history
- Hepatitis B (series of 3 shots)

Do students have to be Seventh-day Adventist to attend?
No. Miami Springs Adventist School welcomes families from many cultural and religious backgrounds. Students do not need to be Adventist to attend, but families should understand and respect the school’s Christ-centered mission, values, and standards.
What grades does MSAS serve?
MSAS serves students in kindergarten through 8th grade.
What is the first step to apply?
The first step is to download and complete the application packet. From there, you gather the required documents and submit health records as listed on this page.
What documents do I need to submit?
Most families submit completed application forms, academic records, recommendation letters, identification documents, health forms, and any applicable testing reports. The full list is grouped above in “What You’ll Need to Apply.”
Are scholarships available?
Yes. Many MSAS families use scholarship programs such as Step Up For Students, Family Empowerment, and AAA Scholarship. Our office can help you understand which programs may fit your family.
